Introduction
Levelheaded Health, LLC ("Levelheaded," "we," or "us") owns and operates the website located at www.livelevelheaded.com (collectively, the "Platform"). Your access and use of the Platform, any part thereof, or anything associated therewith, including its content ("Content"), any products or services provided through the Platform or otherwise by Levelheaded, and any affiliated website, software or application owned or operated by Levelheaded (collectively, including the Platform and the Content, the "Service") are subject to our Privacy Policy unless specifically stated otherwise. Capitalized terms not otherwise defined in our Privacy Policy or this Consumer Health Data Privacy Policy have the same meaning as set forth in the Levelheaded Terms of Use ("Terms of Use").
This Consumer Health Data Privacy Policy applies to the extent required by applicable state law with respect to consumer health data ("Consumer Health Data") as defined by applicable law. This policy does not apply to protected health information subject to the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"); please refer to the Notice of Privacy Practices provided by MDI Medical Group PC, which governs such information. This Consumer Health Data Privacy Policy supplements our general Privacy Policy. In the event of a conflict between our Privacy Policy and this Consumer Health Data Privacy Policy, this Consumer Health Data Privacy Policy applies to the extent it is consistent with applicable state law.
The Consumer Health Data we collect depends on how you interact with us, the services you use, and the choices you make. We collect Consumer Health Data about you from different sources and in various ways when you use our services, including Consumer Health Data you provide directly, Consumer Health Data collected automatically, Consumer Health Data we obtain from third-party sources, and Consumer Health Data we infer or generate from other data.

Identifiers and device information. When you visit our website, our servers automatically log your Internet Protocol (IP) address and information about your device, including device identifiers, device type, and your device's operating system, browser, and other software including type, version, language, settings, and configuration. Levelheaded does not sell Consumer Health Data, as "sell" is defined under applicable state law. Levelheaded does not use geofencing technology to establish a virtual boundary around a mental health facility, reproductive or sexual health facility, or other health care facility for the purpose of identifying, tracking, collecting data from, or sending notifications to consumers regarding their Consumer Health Data.
Your Consumer Health Data Rights
You may have certain rights to your Consumer Health Data under applicable state law. These rights may vary depending on your state of residence and may be subject to certain limitations.
- Withdraw consent. To the extent we rely on your consent for our collection or sharing of your Consumer Health Data, you have the right to withdraw that consent as to future collection or sharing.
- Access and confirm. You have the right to ask us to confirm whether we have collected, shared, or sold your Consumer Health Data, and to access a copy of that data, including a list of third parties and affiliates with whom we have shared or sold it.
